Health Canada advises all Canadians to report unwanted effects experienced from prescribed drugs. Health Canada says:

“[… The] information is used to check for new safety concerns about a product. As a consumer, your report contributes to improving the safety and safe use of health products for everyone.”

This report form does not have to be filled out by the person who took the medication being reported.  It also could be filled out by a friend or family member who has knowledge of what happened, or you can also ask your doctor to report this if you prefer.
